We realize that you DO have a choice, and welcome the opportunity to serve you and your individual needs. Hope Homes is committed to ensuring that your individual needs are treated with the utmost respect. You can stay as long as you choose! Hope Homes provides residential services and adult day programs. Our residential services provide a community home with support staff to meet your individual needs. Adult day programs are held for those who live in their own homes but want to enhance social interaction, participate in stimulating cognitive activities, and enjoy community trips to many local places and events. Please contact Jane Horn, the admissions officer here at Hope Homes, if you have any questions. She will be the one to guide you through the admissions process and answer any questions that you might have along the way. Individuals and/or their family members are encouraged to contact us or visit us in order to learn more about our services. For residential services, Hope Homes accepts applicants through your county's Board of Developmental Disabilities or through private pay. If you are not currently receiving services through the your county's Board of Developmental Disabilities, you can contact them for more information about the referral process. If you are already working with an SSA (Service and Support Administrator, a case manager), you can move on to the Admissions process.
